Objective:
The Pediatric Epilepsy Research Consortium (PERC) Epilepsy Surgery Database Project is a multisite collaborative that includes neuropsychological evaluations of children presenting for epilepsy surgery. There is some evidence for specific neuropsychological phenotypes within epilepsy (Hermann et al, 2016); however, this is less clear in pediatric patients. As a first step, we applied an empirically-based subtyping approach to determine if there were specific profiles using indices from the Wechsler scales [Verbal IQ (VIQ), Nonverbal IQ (NVIQ), Processing Speed Index (PSI), Working Memory Index (WMI)]. We hypothesized that there would be at least four profiles that are distinguished by slow processing speed and poor working memory as well as profiles with significant differences between verbal and nonverbal reasoning abilities.
Participants and Methods:Our study included 372 children (M=12.1 years SD=4.1; 77.4% White; 48% male) who completed an age-appropriate Wechsler measure, enough to render at least two index scores. Epilepsy characteristics included 84.4% with focal epilepsy (evenly distributed between left and right focus) and 13.5% with generalized or mixed seizure types; mean age of onset = 6.7 years, SD = 4.5; seizure frequency ranged from daily to less than monthly; 53% had structural etiology; 71% had an abnormal MRI; and mean number of antiseizure medications was two. Latent profile analysis was used to identify discrete underlying cognitive profiles based on intellectual functioning. Demographic and epilepsy characteristics were compared among profiles.
Results:Based on class enumeration procedures, a 3-cluster solution provided the best fit for the data, with profiles characterized by generally Average, Low Average, or Below Average functioning. 32.8% were in the Average profile with mean index scores ranging from 91.7-103.2; 47.6% were in the Low Average profile with mean index ranging from 80.7 to 84.5; and 19.6% were in the Below Average profile with mean index scores ranging from 55.0-63.1. Across all profiles, the lowest mean score was the PSI, followed by WMI. VIQ and NVIQ represented relatively higher scores for all three profiles. Mean discrepancy between indices within a profile was as large as 11.5 IQ points. No demographics or epilepsy characteristics were significantly different across cognitive phenotypes.
Conclusions:Latent cognitive phenotypes in a pediatric presurgical cohort were differentiated by general level of functioning; however, across profiles, processing speed was consistently the lowest index followed by working memory. These findings across phenotypes suggest a common relative weakness which may result from a global effect of antiseizure medications and/or the widespread impact of seizures on neural networks even in a largely focal epilepsy cohort; similar to adult studies with temporal lobe epilepsy (Hermann et al, 2007). Future work will use latent profile analysis to examine phenotypes across other domains relevant to pediatric epilepsy including attention, naming, motor, and memory functioning. These findings are in line with collaborative efforts towards cognitive phenotyping which is the aim of our PERC Epilepsy Surgery Database Project that has already established one of the largest pediatric epilepsy surgery cohorts.
Peritoneal catheters in neonates undergoing complex cardiac surgery: a multi-centre descriptive study

Background:
The use of peritoneal catheters for prophylactic dialysis or drainage to prevent fluid overload after neonatal cardiac surgery is common in some centres; however, the multi-centre variability and details of peritoneal catheter use are not well described.
Methods:Twenty-two-centre NEonatal and Pediatric Heart Renal Outcomes Network (NEPHRON) study to describe multi-centre peritoneal catheter use after STAT category 3–5 neonatal cardiac surgery using cardiopulmonary bypass. Patient characteristics and acute kidney injury/fluid outcomes for six post-operative days are described among three cohorts: peritoneal catheter with dialysis, peritoneal catheter with passive drainage, and no peritoneal catheter.
Results:Of 1490 neonates, 471 (32%) had an intraoperative peritoneal catheter placed; 177 (12%) received prophylactic dialysis and 294 (20%) received passive drainage. Sixteen (73%) centres used peritoneal catheter at some frequency, including six centres in >50% of neonates. Four centres utilised prophylactic peritoneal dialysis. Time to post-operative dialysis initiation was 3 hours [1, 5] with the duration of 56 hours [37, 90]; passive drainage cohort drained for 92 hours [64, 163]. Peritoneal catheter were more common among patients receiving pre-operative mechanical ventilation, single ventricle physiology, and higher complexity surgery. There was no association with adverse events. Serum creatinine and daily fluid balance were not clinically different on any post-operative day. Mortality was similar.
Conclusions:In neonates undergoing complex cardiac surgery, peritoneal catheter use is not rare, with substantial variability among centres. Peritoneal catheters are used more commonly with higher surgical complexity. Adverse event rates, including mortality, are not different with peritoneal catheter use. Fluid overload and creatinine-based acute kidney injury rates are not different in peritoneal catheter cohorts.

Davidlloydite, ideally Zn3(AsO4)2(H2O)4, is a new supergene mineral from the Tsumeb mine, Otjikoto (Oshikoto) region, Namibia. It occurs as elongated prisms (∼10:1 length-to-width ratio) that are flattened on {010}, and up to 100 × 20 × 10 μm in size. The crystals occur as aggregates (up to 500 μm across) of subparallel to slightly diverging prisms lying partly on and partly embedded in fine-grained calcioandyrobertsite. Crystals are prismatic along [001] and flattened on {010}, and show the forms {010} dominant and {100} subsidiary. Davidlloydite is colourless with a white streak and a vitreous lustre; it does not fluoresce under ultraviolet light. The cleavage is distinct on {010}, and no parting or twinning was observed. The Mohs hardness is 3 – 4. Davidlloydite is brittle with an irregular to hackly fracture. The calculated density is 3.661 g cm–3. Optical properties were measured with a Bloss spindle stage for the wavelength 590 nm using a gel filter. The indices of refraction are α = 1.671, β = 1.687, γ = 1.695, all ±0.002; the calculated birefringence is 0.024; 2Vobs = 65.4(6)°, 2Vcalc = 70°; the dispersion is r < v, weak; pleochroism was not observed. Davidlloydite is triclinic, space group P1, with a = 5.9756(4), b = 7.6002(5), c = 5.4471(4) Å, α = 84.2892(9), β = 90.4920(9), γ = 87.9958(9)°, V = 245.99(5) Å3, Z = 1 and a:b:c = 0.7861:1:0.7167. The seven strongest lines in the X-ray powder diffraction pattern [listed as d (Å), I, (hkl)] are as follows: 4.620, 100, (011, 10); 7.526, 71, (010); 2.974, 49, (200, 01); 3.253, 40, (021, 120); 2.701, 39, (10, 002, 1); 5.409, 37, (001); 2.810, 37, (210). Chemical analysis by electron microprobe gave As2O5 43.03, ZnO 37.95, CuO 5.65, H2O(calc) 13.27, sum 99.90 wt.%. The H2O content and the valence state of As were determined by crystal structure analysis. On the basis of 12 anions with H2O = 4 a.p.f.u., the empirical formula is (Zn2.53Cu0.39)Σ2.92As2.03O8(H2O)4.
The crystal structure of davidlloydite was solved by direct methods and refined to an R1 index of 1.51% based on 1422 unique observed reflections collected on a three-circle rotating-anode (MoKα radiation) diffractometer equipped with multilayer optics and an APEX-II detector. In the structure of davidlloydite, sheets of corner-sharing (As5+O4) and (ZnO4) tetrahedra are linked by ZnO2(H2O)4 octahedra. The structure is related to that of parahopeite.

Schlüterite-(Y), ideally (Y,REE)2Al(Si2O7)(OH)2F, is a new silicate mineral species from the Stetind pegmatite, Tysfjord, Nordland, Norway. It forms dense, fibrous, radiating aggregates (up to ∼2 mm) diverging to individual needle-like crystals (up to ∼1 mm long) in cavities. Crystals are acicular to bladed, flattened on {001} and elongated along [010], and the dominant form is {001}. Schlüterite-(Y) is transparent, pale pink with a white streak and a vitreous lustre, and does not fluoresce under short-wave ultraviolet light. Mohs hardness is 5½–6, and schlüterite-(Y) is brittle with an irregular fracture, and has no cleavage. The calculated density is 4.644 g/cm3. The indices of refraction are α = 1.755, β = 1.760, γ = 1.770, all ± 0.005, 2Vobs = 71.8 (5)°, 2Vcalc = 71°, non-pleochroic, optic orientation is X ˆ a = 83.1° (β obtuse), Y//b, Z ˆ c = 50.3° (β acute). Schlüterite-(Y) is monoclinic, space group P21/c, a 7.0722(2), b 5.6198(1), c 21.4390(4) Å, β 122.7756(3)°, V 716.43(5) Å3, Z = 4. The seven strongest lines in the X-ray powder-diffraction pattern are as follows: [d (Å), I, (hkl)]: 4.769, 100, (012); 2.972, 55, (14); 3.289, 51, (112); 2.728, 49, (16); 2.810, 37, (020); 3.013, 37, ((16); 4.507, 36, (004). Chemical analysis by electron microprobe gave SiO2 22.64, Al2O3 9.45, Y2O3 15.35, La2O3 3.25, Ce2O3 9.69, Pr2O3 2.05, Nd2O3 9.50, Sm2O3 3.57, Gd2O3 4.65, Dy2O3 4.21, Er2O3 2.31, Yb2O3 1.86, F 2.71, H2Ocalc 3.78, O = F −1.14, sum 93.88 wt%. The H2O content was determined by crystal-structure analysis. On the basis of 10 anions with (OH) + F = 3 a.p.f.u. (atoms per formula unit), the empirical formula is (Y0.73Ce0.32Nd0.30Gd0.14Dy0.12La0.11Sm0.11Pr0.07Er0.06Yb0.05)Σ=2.01Al0.99Si2.01O7(OH)2.24F0.76. The crystal structure of schlüterite-(Y) was solved by direct methods and refined to an R1 index of 1.8% based on 1422 unique observed reflections. In the structure of schlüterite-(Y), Al(OH)4O2 octahedra share (OH)–(OH) edges to form [MΦ4] chains that are decorated by (Si2O7) groups that bridge O vertices of neighbouring octahedra in a staggered fashion on either side of the chain. These [Al(OH)2(Si2O7)] chains extend parallel to b, and are linked into a continuous framework via bonds to interstitial [8](Y,REE) (= <2.400 Å>) and [9](Y,REE) (= <2.548 Å>) atoms.

Krásnoite is a new mineral (IMA2011-040) from the Huber open pit, Krásno ore district, Czech Republic and the Silver Coin mine, Nevada, USA. Krásnoite is the fluorophosphate analogue of perhamite. Krásnoite occurs as compact to finely crystalline aggregates, balls and rosette-like clusters up to 1 mm across. Individual crystals are platy, show a hexagonal outline and can reach 0.1 mm on edge at Krásno and 0.4 mm at Silver Coin. At both localities, krásnoite occurs very late in phosphaterich paragenetic sequences. Krásnoite crystals are partly transparent with a typically pearly lustre, but can also appear greasy (Krásno) or dull (Silver Coin). The streak is white and the hardness is 5 on the Mohs scale. Crystals are brittle, have an irregular fracture, one imperfect cleavage on {001} and are not fluorescent under SW and LW ultraviolet light. Penetration twinning ⊥ {001} is common. The density for both Krásno and Silver Coin material is 2.48(4) g cm–3, measured by the sink–float method in an aqueous solution of sodium polytungstate. The calculated density is 2.476 g cm–3 (Krásno). Krásnoite crystals are uniaxial (+), with ω = 1.548(2) and ε = 1.549(2) (Krásno) and ω = 1.541(1) and ε = 1.543(1) (Silver Coin). The simplified formula of krásnoite is: Ca3Al7.7Si3P4O23.5(OH)12.1F2·8H2O. Krásnoite is trigonal, space group Pm1, with a = 6.9956(4), c = 20.200(2) Å, V = 856.09(9) Å3 and Z = 3. Raman and infrared spectroscopy, coupled with magic-angle spinning nuclear magnetic resonance (MAS–NMR) spectrometry, confirmed the presence of PO3F, PO4, SiO4, H2O and OH in the crystal structure of krásnoite.

Ianbruceite, ideally [Zn2(OH)(H2O)(AsO4)](H2O)2, is a new supergene mineral from the Tsumeb mine, Otjikoto (Oshikoto) region, Namibia. It occurs as thin platy crystals up to 80 μm long and a few μm thick, which form flattened aggregates up to 0.10 mm across, and ellipsoidal aggregates up to 0.5 mm across. It is associated with coarse white leiteite, dark blue köttigite, minor legrandite and adamite. Ianbruceite is sky blue to very pale blue with a white streak and a vitreous lustre; it does not fluoresce under ultraviolet light. It has perfect cleavage parallel to (100), is flexible, and deforms plastically. The Mohs hardness is 1 and the calculated density is 3.197 g cm-3. The refractive indices are α = 1.601, β = 1.660, γ = 1.662, all ±0.002; 2Vobs = 18(2)°, 2Vcalc = 20°, and the dispersion is r < v, weak. Ianbruceite is monoclinic, space group P21/c, a = 11.793(2), b = 9.1138(14), c = 6.8265(10) Å, β = 103.859(9)°, V = 712.3(3) Å3, Z = 4, a:b:c = 1.2940:1:0.7490. The seven strongest lines in the X-ray powder diffraction pattern [d(Å), I, (hkl)] are as follows: 11.29, 100, (100); 2.922, 17, (130); 3.143, 15, (202); 3.744, 11, (300); 2.655, 9, (230); 1.598, 8, (152); 2.252, 7, (222). Chemical analysis by electron microprobe gave As2O5 36.27, As2O3 1.26, Al2O3 0.37, ZnO 49.72, MnO 0.32, FeO 0.71, K2O 0.25, H2Ocalc 19.89, sum 108.79 wt.%; the very high oxide sum is due to the fact that the calculated H2O content is determined from crystal-structure analysis, but H2O is lost under vacuum in the electron microprobe.
The crystal structure of ianbruceite was solved by direct methods and refined to an R1 index of 8.6%. The As is tetrahedrally coordinated by four O anions with a mean As O distance of 1.687 Å. Zigzag [[5]Zn[6]Znϕ7] chains extend in the c direction and are linked in the b direction by sharing corners with (AsO4) tetrahedra to form slabs with a composition [Zn2(OH)(H2O)(AsO4)]. The space between these slabs is filled with disordered (H2O) groups and minor lone-pair stereoactive As3+. The ideal formula derived from chemical analysis and crystal-structure solution and refinement is [Zn2(OH)(H2O)(AsO4)](H2O)2.
Outbreak of Klebsiella pneumoniae Carbapenemase–Producing Citrobacter freundii at a Tertiary Acute Care Facility in Miami, Florida

OBJECTIVE
To describe the investigation and control of a rare cluster of Klebsiella pneumoniae carbapenemase–producing Citrobacter freundii in a hospital in southern Florida.
METHODSAn epidemiologic investigation, review of infection prevention procedures, and molecular studies including whole genome sequencing were conducted.
RESULTSAn outbreak of K. pneumoniae carbapenemase–3-producing C. freundii was identified at a tertiary hospital in Florida in 2014. Of the 6 cases identified, 3 occurred in the same intensive care unit and were caused by the same clone. For 2 of the 3 remaining cases, the isolates had low carbapenem minimum inhibitory concentrations and were unrelated by whole genome sequencing. As a response to the outbreak, supplementary environmental cleaning was implemented, including closure and terminal cleaning of the unit where the 3 cases clustered, in addition to the infection control bundle already in place at the time. No further cases were identified after these additional interventions.
CONCLUSIONSAlthough C. freundii is not a species that commonly demonstrates carbapenem resistance, our findings suggest that carbapenemase-producing C. freundii may be underdetected even when active surveillance is in place and has a potential to cause hospital outbreak.

Effect of vitamin D deficiency during pregnancy on offspring bone structure, composition and quality in later life

During foetal development, calcium requirements are met as a consequence of maternal adaptations independent of vitamin D status. In contrast, after birth, dependency on vitamin D appears necessary for calcium metabolism and skeletal health. We used a rodent model (Sprague-Dawley rats), to determine if maternal vitamin D deficiency during pregnancy had a deleterious effect on bone structure at birth. Vitamin D deplete females were maintained under deplete conditions until birth of the pups, whereupon all dams were fed a vitamin D replete diet. Offspring were harvested at birth, and 140 days of age. Bones were analyzed using micro-computed tomography and strength tested to study differences in bone structure, density and strength and subjected to elemental analysis using plasma mass spectrometry to determine strontium, barium and calcium contents. Offspring from deplete mothers displayed altered trabecular parameters in the femur at birth and 140 days of age. In addition, at 140 days of age there was evidence of premature mineralization of the secondary ossification centre of the femoral head. Elemental analysis showed increased strontium uptake in the femur of the developmentally vitamin D-deficient offspring. Vitamin D depletion during development in the offspring may have a long-lasting effect, despite repletion of vitamin D from birth. This may have consequences for human health given the low vitamin D levels seen during pregnancy and current lifestyle of sun avoidance due to the risk of skin cancer.

Consistent positive relationships have been found between birth weight and grip strength in adults but evidence in children is limited. In a prospective general population birth cohort (Southampton Women's Survey), grip strength and anthropometry (height and weight) were measured in 968 children at the age of 4 years. Mean (standard deviation (s.d.)) birth weight was 3.48 (0.52) kg. Birth weight, adjusted for sex and gestational age, was positively associated with grip strength (β = 0.22 kg/s.d. increase in adjusted birth weight; 95% CI 0.11, 0.34). The relationship was attenuated after adjustment for current height and weight such that it became non-significant (β = 0.03 kg/s.d. increase in adjusted birth weight; 95% CI−0.08, 0.14), suggesting that body size may be on the causal pathway. Early influences on muscle development appear to impact on grip strength in children, as well as adults.

In the summer of 2009, an outbreak of verocytotoxigenic Escherichia coli O157 (VTEC O157) was identified in visitors to a large petting farm in South East England. The peak attack rate was 6/1000 visitors, and highest in those aged <2 years (16/1000). We conducted a case-control study with associated microbiological investigations, on human, animal and environmental samples. We identified 93 cases; 65 primary, 13 secondary and 15 asymptomatic. Cases were more likely to have visited a specific barn, stayed for prolonged periods and be infrequent farm visitors. The causative organism was identified as VTEC O157 PT21/28 with the same VNTR profile as that isolated in faecal specimens from farm animals and the physical environment, mostly in the same barn. Contact with farm livestock, especially ruminants, should be urgently reviewed at the earliest suspicion of a farm-related VTEC O157 outbreak and appropriate risk management procedures implemented without delay.

The photophysics and nonlinear absorption behaviour of a series of α, ω-dithienyl polyenes were studied in chloroform and n-octane solutions. Steady state fluorescence, fluorescence lifetime and quantum yield measurements were made. The fluorescence quantum yields were significantly lower than those of the corresponding ax,o-diphenyl polyenes. Among the dithienyl polyenes, 3,3′-substituted polyenes exhibited stronger fluorescence than those of the 2,2′-substituted. Nonlinear optical absorption experiments showed evidence of two photon absorption.

The great interest generated by the theme of this year’s conference reflects the central importance of children in the history of the Christian Church, yet at the same time their omission from much of historical writing. For all but the recent past this is largely the result of the difficulties with the source material itself, and this is certainly true for historians of the Church during the medieval and Reformation periods. The main concern of the administrative records of the Catholic Church was with adults and, in particular, ordained men. It is to the schools that we must look for the most useful references to children and, more specifically, to the choir schools for evidence of the role of boys in the liturgy.

Hormone treatment for impotence and impotentia ejaculandi (delayed or absent ejaculation in the presence of normal erections and desire) (Johnson, 1965) is based on two assumptions: namely—that (1) potency in the male is androgen dependent and (2) impotency is due to relative androgen deficiency (Miller, 1968; Margolis et al., 1967; Sobotka, 1969). The synthetic preparations most frequently used are methyl testosterone, various esters, including the propionate, deconate, enanthate, etc. (Tuthill, 1955; Kupperman, 1967) and more recently a number of testosterone- ‘aphrodisiac’ combinations (Margolis et al., 1967; Cooper et al., 1971, etc.). Despite some claims of therapeutic success for the various regimes, the administration of exogenous hormones has several pharmacologic disadvantages. For instance methyl testosterone, which has to be taken sublingually or bucally, may be associated with severe toxic effects such as cholestatic jaundice, etc. (Goodman and Gilman, 1965). Parenterally administered testosterone esters, although less toxic and more certainly absorbed than methyl testosterone, have all the drawbacks of the intramuscular route; also their rate(s) of tissue release may not be optimal for metabolism. Additionally, all these synthetic compounds may inhibit endogenous testosterone formation and spermatogenesis by suppressing pituitary gonadotrophins. Long term administration may result in Leydig cell inactivity and even disuse atrophy. Recently Harkness et al. (1964, 1968) have observed that clomiphene (Fig. 1), a non-steroid triethylene derivative which in females stimulates the anovulatory ovary to secrete oestrogens (Bishop, 1970), significantly raised the levels of urinary steroid metabolites, including testosterone, in a proportion of ‘normal’ and corticosteroid deficient males. This unexpected testosterone elevating effect suggested its possible ‘experimental’ use as an alternative drug treatment in impotence. Accordingly a limited pilot study on five ‘primary’ (non-organic) impotent males was conducted: its main aims were (a) to assess the potential clinical utility of the drug and (b) to study its effects on the ‘pituitary-testes-axis' as reflected by plasma testosterone levels.
